WRITTEN APPEALS

In order to take appropriate measures to protect, ensuring the implementation, restoration of rights, freedoms and (or) legitimate interests of applicants, the institution has a procedure for considering written appeals.

Written applications of applicants must meet the requirements, set out in paragraphs 1, 2 articles 12 Law of the Republic of Belarus dated 18 july 2011 g. No. 300-З "On appeals of citizens and legal entities" (with changes and additions) (further - Law):

1. Appeals are presented in Belarusian or Russian.
2. Written appeals of citizens must contain:

• Name and (or) address of the organization or position and (or) last name, given name, surname (if any) or initials of the person, to whom the appeal is directed;
• last name, given name, surname (if any) or the initials of the citizen, address of his residence (places of stay);
• statement of the essence of the appeal;
• personal signature of a citizen (citizens).

3. Written appeals of legal entities must contain:

• Name and (or) address of the organization or position and (or) last name, given name, surname (if any) or initials of the person, to whom the appeal is directed;
• full name of the legal entity and its location;
• statement of the essence of the appeal;
• last name, given name, surname (if any) or the initials of the head or person, duly authorized to sign appeals;
• personal signature of the head or person, duly authorized to sign appeals, certified by the seal of a legal entity.

4. Remarks and (or) suggestions are made in the book of comments and suggestions in accordance with the form of the book of comments and suggestions, established by the Council of Ministers of the Republic of Belarus.
5. The text of the appeal must be readable. Handwritten messages must be written clearly, legible handwriting. It is not allowed to use obscene or offensive words or expressions in appeals.
6. In cases, when a citizen does not have the opportunity to personally apply with an offer, statement or complaint, he can entrust another person to do it on his behalf.

• An appeal on behalf of an incapacitated citizen or recognized as having limited legal capacity is submitted by his legal representative. Representatives of the applicants exercise their powers on the basis of legislative acts, or acts of authorized state bodies, or power of attorney, arranged in order, established by civil law.

For written requests, submitted by the representatives of the applicants, attached documents, confirming their authority.

7. Applications should contain information on the results of their previous consideration with an attachment (in the presence of) documents confirming this information.

Upon receipt of a written request to the institution "Gomel Regional Center for Preventive Disinfection", containing questions, the decision of which is not within its competence, а также в случае поступления письменного обращения в учреждение «Гомельский областной центр профилактической дезинфекции» без соблюдения порядка рассмотрения, established by the Decree of the President of the Republic of Belarus dated 15 October 2007 № 498 "On Additional Measures for Working with Appeals of Citizens and Legal Entities" (with changes and additions) (hereinafter - Decree), this appeal is sent within five working days for consideration to the authorities or organizations, whose competence includes the consideration of this appeal, about which the applicant is notified on the day of sending to the body or organization, or at the same time in the order, established by law, leaves the appeals without consideration on the merits and notifies the applicant about this with an explanation, which organization and in what order should be contacted to resolve issues, set out in the circulation.

Written appeals may be left without consideration on the merits in the manner and in cases, certain articles 15 of the Law:

• not presented in Belarusian or Russian;
• do not contain last name, own name, patronymic (if any) or initials, residence addresses (places of stay) citizen;
• the essence of the appeal is not stated;
• does not contain a personal signature of a citizen (citizens);
• does not include a name and (or) address of the organization or position of the person, to whom the appeal is directed;
• does not contain the full name of the legal entity and its location;
• does not contain last name, given name, surname (if any) or the initials of the head or person, duly authorized to sign appeals;
• does not contain the personal signature of the head or person, duly authorized to sign appeals, certified by the seal of a legal entity;
• to written requests, submitted by the representatives of the applicants, no documents attached, confirming their authority;
• contain text, unreadable;
• contain obscene or offensive words or expressions;
• documents not attached, confirming credentials (in the case of filing an application by the representatives of the applicants);
• documents not attached;
• appeals are subject to consideration in accordance with the legislation on constitutional proceedings, civil, civil procedural, economic procedural, criminal procedure legislation, legislation, determining the order of the administrative process, the legislation on administrative procedures or in accordance with legislative acts establishes a different procedure for the submission and consideration of such appeals;
• contain questions, not falling within the competence of the state institution "Gomel Regional Center for Preventive Disinfection";
• Missed without good reason the deadline for filing a complaint;
• re-application filed, including those entered in the book of comments and suggestions, if it has already been considered on the merits and does not contain new circumstances, important for consideration of the appeal on the merits;
• correspondence with the applicant was terminated.
• appeals contain threats to life, health and property, incitement to commit an unlawful act or the applicant otherwise abuses the right to appeal

The applicant has the right to withdraw his appeal before considering it on the merits by submitting an appropriate written application.

В случае отзыва заявителем своего обращения учреждение «Гомельский областной центр профилактической дезинфекции» прекращает рассмотрение этого обращения по существу и возвращает заявителю оригиналы документов, attached to circulation.

Anonymous calls, that is, the applicants' appeals, in which the last name is not indicated, given name, surname (if any) either the initials of the citizen or the address of his place of residence (places of stay) or the name of the legal entity (complete or abbreviated) or its location or the specified data do not correspond to reality, are not subject to consideration, if they do not contain information about the forthcoming, committed or committed crime.

Appeals, containing information about upcoming, committed or committed crime or other offense, no later than five working days sent by organizations, in which they entered, to the appropriate law enforcement or other government agencies.

The response of the state institution "Gomel Regional Preventive Disinfection" to the appeal or the decision to leave the appeal without consideration on the merits can be appealed to a higher organization (State Institution "Gomel Regional Center for Hygiene, Epidemiology and Public Health ", g. Gomel, st. Moiseenko, 49).
